Oregon Family Planning Program Manual
The Oregon Family Planning Program Manual is divided into six sections. Clinics may need some or all of them, depending on whether they are FPEP-only or FPEP and Title X, and if they participate in the IPP project. All clinics should have:

As of October 2008, these materials have been incorporated into the Family Planning Manual.
Please see Section A.7, as well as Exhibits A3 through A7, for details on providing
vasectomies through FPEP orTitle X.
For your convenience the consent to sterilization forms are provided here.
All other forms for vasectomies are exclusively for Title X and located in Section A Exhibits 5 and 6.

Cost Analysis Training with Gerry Christie (2007) - We recommend viewing the full series in date order.
1. Download ("save") the Conference Player (exe) to your Desktop.
2. Download the audio recordings to your Desktop (see the .nca files below).
3. Go to your Desktop and double-click the ConferencePlayerSetup.exe file to install the player.
4. Open the recording .nca files to play the recordings.

Eligibility Changes 2006
As a result of the federal Deficit Reduction Act of 2005, all clients must provide citizenship verification documents to be eligible for FPEP (Family Planning Expansion Project), Oregon's family planning Medicaid waiver.
Out-of-State Birth Certificate Requests
FPEP applicants born in Oregon may request an electronic search for their birth record by submitting a Birth Information Form to their local clinic. Click here for more information about Oregon Birth Record Requests.
As of March 1, 2008, the State Family Planning Program will order birth certificates from other states to help non-Oregon born FPEP applicants document U.S. citizenship. The materials and links below explain this process.

How to Become an FPEP Provider
- To start the enrollment process, read through the Provider Enrollment Memo (PDF) and the Prospective Provider Packet (PDF) to determine if your clinic can comply with FPEP program requirements. If so, then download and complete these forms:
- Mail or deliver the completed Application and Project/Site Form to the address on the Application.
- Next, download and read through the FPEP Manual and associated Exhibits, and the FPEP Administrative Rules (PDF).
